Request Transcript PDF
A transcript is an official copy of the student’s record showing units and grades of SFCM work. The Conservatory can not issue transcripts of work completed at any other school.
How to Order Your Transcript:
1. In Person
Pick up a Transcript Request Form during business hours from the Registrar's Office Room #408 or download the Transcript Request PDF file. You will have to make your payment at the Business Office. They are closed in the afternoon 12:30-1:30 for lunch.
2. By Mail
Download the Transcript Request PDF and mail it with payment to:

3. By Fax
Download the Transcript Request PDF and fax to: 415-503-6299
Make sure you indicate if you are sending a check/money order or paying by credit card.
4. Online
Fill out the online request form and follow payment option instructions.
Payment Options:
The first copy ever ordered of your transcript is issued free. The charge for each additional copy is $10.00. The transcript will not be sent until the receipt of payment. If you are in a rush please read the Federal Express option.

Please indicate “for transcripts” somewhere on the check.
Federal Express
Transcripts can be delivered through FedEx for domestic and international locations. Charges are based on location and type of service. Be aware Federal Express cannot deliver to a P.O. Box. Please indicate on your transcript request form when you would like your transcript to arrive and we will choose the appropriate service. You must supply a credit card number with expiration date or a FedEx account number. You will be charged by FedEx. For arrival and cost estimates you can also visit their web site at
www.fedex.com.
You may pay by submitting your Federal Express account number, MasterCard or Visa account number and expiration date, or by check. When paying by check, please write a separate check out to “The San Francisco Conservatory of Music” with “Federal Express Reimbursement” written on the check.
